Tech Neck Syndrome: How Screen Time Affects the Spine and Muscles

Smartphones, laptops, tablets, and desktop computers have become an essential part of modern life. People use screens for work, study, communication, entertainment, shopping, travel, and even exercise. However, spending long periods looking down at a phone or leaning towards a computer may contribute to neck stiffness, shoulder tension, headaches, upper-back discomfort, and reduced movement.

These symptoms are often described as tech neck syndrome or text neck.

Tech neck is not a specific structural diagnosis. It is a commonly used term for neck and upper-body symptoms associated with prolonged screen use, sustained positions, limited movement, unsuitable workstation setup, and repeated loading of the neck and shoulder muscles.

Research shows an association between smartphone use and musculoskeletal symptoms, particularly around the neck and shoulders. However, current evidence does not prove that looking down at a phone automatically damages the cervical spine or that one particular posture is solely responsible for neck pain. Sleep, stress, physical activity, muscle endurance, previous pain, workstation design, screen duration, and movement habits may all contribute.

The important issue is therefore not simply whether your head moves forward. It is how long you remain in one position, how often you move, how well your muscles tolerate the workload, and whether your daily environment supports comfortable movement.

What Is Tech Neck Syndrome?

Tech neck syndrome describes a group of symptoms that may develop when the neck, upper back, shoulders, and arms are exposed to prolonged or repetitive screen-related positions.

A common example is sitting with:

·         The head positioned forward

·         The chin dropping towards the chest

·         The upper back rounded

·         The shoulders moving forward

·         The arms unsupported

·         The phone resting near the lap

·         The laptop positioned below eye level

The human neck is designed to move in several directions. Looking down for a short period is usually not a problem. Difficulties are more likely to develop when a position is held repeatedly without sufficient movement or recovery.

During prolonged screen use, the muscles around the neck and shoulder blades may need to work continuously to control the head. As this demand continues, some muscles may become fatigued, overactive, or sensitive, while others may provide less effective support.

Tech neck can affect office professionals, students, gamers, healthcare workers, drivers, content creators, online teachers, designers, traders, and anyone who spends significant time using digital devices.

How Screen Time Affects the Cervical Spine

The cervical spine is formed by seven vertebrae in the neck. These joints work together with discs, ligaments, nerves, and muscles to support the head and allow movement.

Increased Mechanical Demand During Neck Flexion

When the head moves forward or bends down, the distance between the weight of the head and the cervical joints increases. This creates a greater mechanical demand on the muscles that support the neck.

Biomechanical research has shown that greater neck-flexion angles during smartphone use can increase the gravitational moment acting on the cervical region and alter neck-muscle activity. This does not mean the spine is being permanently damaged. It means the muscles and joints may need to work harder while the position is maintained.

For a short task, the body can usually manage this load. During prolonged texting, scrolling, gaming, reading, or laptop work, the sustained demand may contribute to fatigue and discomfort.

Reduced Movement Variability

The body generally tolerates movement better than prolonged stillness. Even a posture that appears ideal may become uncomfortable when held for several hours.

Screen use often reduces natural movement because attention remains fixed on the device. People may sit without changing position, rotating the neck, moving the shoulders, or standing up.

This reduced movement variability may lead to:

·         Joint stiffness

·         Reduced neck rotation

·         Upper-back tightness

·         Shoulder fatigue

·         Reduced circulation within continuously active muscles

·         Increased sensitivity around the neck and shoulder region

The concern is therefore not one imperfect posture. The concern is remaining in the same position for too long.

Changes in the Upper Back and Shoulder Position

Screen-related posture does not affect only the neck. The cervical spine, thoracic spine, shoulder blades, rib cage, and arms function as one connected movement system.

When the upper back remains rounded and the shoulders stay forward, the shoulder blades may rest in a different position. The muscles that control the scapulae must then work differently during typing, reaching, lifting, or overhead movement.

This may contribute to:

·         Upper-back stiffness

·         Tightness across the chest

·         Reduced thoracic extension

·         Shoulder-blade discomfort

·         Reduced overhead movement

·         Neck tension during arm activity

A person may feel pain in the neck even when the contributing restriction is partly located in the upper back, chest, or shoulder region.

How Screen Time Affects the Muscles

Cervical Extensor Fatigue

The muscles at the back of the neck help control the head when it moves forward. During prolonged device use, these muscles may remain active for extended periods.

An experimental study examining smartphone-use duration found that longer use was associated with increasing muscle fatigue and discomfort. This supports the importance of screen breaks, position changes, and appropriate device placement.

Upper Trapezius and Shoulder Tension

The upper trapezius connects the neck, shoulder, and upper back. It may become more active when:

·         The arms are unsupported

·         The shoulders are elevated

·         The keyboard is positioned too high

·         The phone is held for long periods

·         The person leans towards the screen

·         Work is performed under stress

Research comparing people with and without neck symptoms found that symptomatic smartphone users demonstrated greater neck-and-shoulder muscle activity during texting tasks.

This can create a feeling of heaviness, tightness, burning, or tenderness across the shoulders.

Reduced Deep Neck Muscle Endurance

The deeper muscles at the front of the neck help control small cervical movements and support head position.

When the head remains forward for prolonged periods, larger surface muscles may begin contributing more than necessary, while the deeper stabilising muscles may not work efficiently. This does not mean the deeper muscles have permanently “switched off,” but their endurance and coordination may need attention during rehabilitation.

Levator Scapulae and Upper-Back Tightness

The levator scapulae connects the upper cervical spine to the shoulder blade. It is frequently involved in screen-related neck and shoulder discomfort, especially when one shoulder is raised, the phone is held to one side, or the head remains rotated while working.

Tightness may be felt:

·         Between the neck and shoulder

·         Near the upper-inner border of the shoulder blade

·         During neck rotation

·         When looking down

·         After carrying a laptop bag

·         After prolonged mouse use

Chest and Shoulder Imbalance

Prolonged sitting with the shoulders forward may increase tightness around the pectoral muscles while reducing the efficiency of muscles that support the shoulder blades.

This may affect shoulder movement and contribute to neck compensation. In some people, improving thoracic mobility and scapular control is therefore as important as treating the neck itself.

Common Signs of Tech Neck Syndrome

Symptoms can vary between individuals. Common concerns include:

·         Neck pain or stiffness

·         Tightness across the shoulders

·         Upper-back discomfort

·         Reduced ability to turn the head

·         Headaches beginning near the neck or base of the skull

·         Burning or aching between the shoulder blades

·         Muscle fatigue while working

·         Discomfort that increases during phone or laptop use

·         Shoulder or arm heaviness

·         Tingling or numbness in the arm or hand

·         Jaw tension

·         Difficulty maintaining an upright sitting position

·         Symptoms after gaming, studying, scrolling, or computer work

Some people feel better after moving, stretching, or changing position. Others experience symptoms that continue after screen use has ended.

Why Some Screen Users Develop Pain and Others Do Not

There is no single posture that explains every case of neck pain.

Two people may use a phone for the same number of hours but experience very different symptoms. This is because pain is influenced by several interacting factors.

These may include:

·         Total screen duration

·         Frequency of movement breaks

·         Muscle strength and endurance

·         Previous neck or shoulder symptoms

·         Workstation design

·         Sleep quality

·         Stress and mental workload

·         Physical activity level

·         Vision problems that cause leaning towards the screen

·         Use of multiple devices

·         Laptop or phone position

·         Repetitive keyboard and mouse activity

·         General health and recovery

A 2025 review evaluating the proposed relationship between “text neck” posture and neck pain concluded that current research does not establish a direct causal relationship. Another prospective study found that cervical-flexion posture during smartphone use was not an independent risk factor for neck pain, while factors such as sleep quality and physical activity also appeared relevant.

This evidence is important because it avoids blaming one posture or creating unnecessary fear around normal spinal movement.

The goal should not be to hold the neck rigidly straight throughout the day. The goal is to create a comfortable setup, develop physical capacity, vary positions, and move regularly.

Who May Be More Likely to Experience Tech Neck Symptoms?

Tech neck symptoms may be more common among people who:

·         Work on laptops for extended periods

·         Use phones near waist or lap level

·         Study online for several hours

·         Work from a sofa, bed, or dining table

·         Use a laptop without an external keyboard

·         Spend long periods gaming

·         Have limited upper-back mobility

·         Have reduced neck or shoulder endurance

·         Frequently work under stress

·         Use screens without regular movement breaks

·         Have recently increased their screen time

·         Wear unsuitable glasses or struggle to see the screen clearly

·         Alternate between long desk hours and intense gym training

Systematic research in younger populations has found an association between prolonged sedentary or screen-based behaviour and neck pain, although the strength of this relationship varies across studies.

When to Consider a Physiotherapy Assessment

A physiotherapy assessment may be helpful when:

·         Neck or shoulder symptoms keep returning

·         Pain affects work, study, driving, or sleep

·         Movement feels increasingly restricted

·         Headaches regularly begin after screen use

·         Symptoms spread towards the arm

·         Muscle tightness returns despite stretching

·         You need repeated pain medication to complete desk work

·         Your workstation feels uncomfortable despite adjustments

·         Symptoms affect gym training or daily activity

·         You are unsure which exercises are appropriate

A physiotherapist can assess whether symptoms are mainly related to muscular fatigue, joint stiffness, movement sensitivity, nerve involvement, shoulder mechanics, upper-back restriction, or another condition.

Symptoms Requiring Medical Attention

Seek prompt medical assessment when neck pain is associated with:

·         Significant trauma, such as a fall or road accident

·         Progressive weakness in an arm or leg

·         Increasing difficulty walking or maintaining balance

·         Severe or constant numbness

·         Loss of hand coordination

·         Fever with severe neck stiffness

·         New bladder or bowel changes

·         Severe unexplained pain that does not change with movement

These symptoms are not typical features of uncomplicated screen-related neck discomfort and may require medical investigation.

How Physiotherapy May Help Tech Neck Syndrome

Physiotherapy does not focus only on making a person “sit straight.” It examines how the whole movement system is responding to screen-related demands.

1. Detailed Posture and Movement Assessment

The physiotherapist may assess:

·         Cervical movement

·         Upper-back mobility

·         Shoulder-blade control

·         Muscle flexibility

·         Neck-muscle endurance

·         Upper-limb strength

·         Nerve-related symptoms

·         Breathing pattern

·         Workstation setup

·         Phone and laptop habits

The purpose is to identify the factors relevant to the individual rather than assuming every case has the same cause.

2. Neck and Upper-Back Mobility

Guided movement may be used to improve stiffness in the cervical and thoracic spine.

·         Middle and lower trapezius

·         Serratus anterior

·         Rotator-cuff muscles

·         Thoracic extensors

·         Shoulder external rotators

·         Upper-limb endurance

Improving shoulder support may reduce the workload placed on the neck during typing, mouse use, lifting, and prolonged sitting.

5. Manual Therapy and Soft-Tissue Techniques

Depending on assessment findings, physiotherapy may include:

·         Joint mobilization

·         Soft-tissue techniques

·         Myofascial release

·         Trigger-point techniques

·         Assisted stretching

·         Movement-based manual therapy

Manual techniques may help manage pain and stiffness, but they are generally more useful when combined with active exercise, education, and habit modification.

A 2023 systematic review found that combining manual therapy with exercise may improve pain and disability in people with nonspecific neck pain, although the certainty of evidence varied across comparisons.

6. Exercise-Based Rehabilitation

Exercise is an important component of neck-pain rehabilitation. Depending on the person, the programme may include resistance exercise, motor-control training, mobility work, breathing exercises, and general physical activity.

Systematic reviews have found that exercise therapy can improve pain and disability in chronic nonspecific neck pain. Research also suggests that several exercise approaches may be useful, with no single exercise type being appropriate for everyone.

7. Workstation and Device Guidance

A physiotherapist may recommend changes such as:

·         Raising the laptop with a stand

·         Using an external keyboard and mouse

·         Keeping the main monitor directly in front

·         Positioning the top of the monitor at or slightly below eye level

·         Supporting the forearms

·         Keeping the mouse close to the body

·         Supporting the feet and lower back

·         Increasing text size instead of leaning forward

·         Using a phone stand during long calls or video viewing

·         Alternating between sitting and standing tasks

OSHA workstation guidance recommends placing the monitor at a height that allows viewing without repeatedly bending the head or neck and positioning it at approximately arm’s length, adjusted for the individual’s vision and task.

Ergonomic changes should not be treated as a requirement to remain perfectly still. Even a well-designed workstation works better when combined with regular movement.

8. Screen-Time and Movement Planning

For many people, completely reducing screen use is unrealistic. A more practical approach is to improve how screen time is organised.

This may include:

·         Changing position regularly

·         Standing during phone calls

·         Looking away from the screen between tasks

·         Moving the neck and shoulders gently

·         Dividing long work into shorter blocks

·         Alternating computer work with walking or standing tasks

·         Keeping recreational screen habits separate from work when possible

·         Increasing general weekly physical activity

WHO guidelines recommend limiting sedentary time and replacing some sedentary behaviour with physical activity of any intensity where possible.

Evidence Shows Improvement

Research on tech neck itself is still developing, and not every study uses the same definition. However, several important findings are consistent:

·         Longer uninterrupted smartphone use may increase neck-muscle fatigue and discomfort.

·         Greater neck-flexion angles can increase mechanical demand and alter cervical muscle activity.

·         People with neck symptoms may demonstrate increased neck-and-shoulder muscle activity during smartphone tasks.

·         Posture alone has not been proven to cause neck pain or structural spinal damage.

·         Exercise therapy can support improvements in chronic nonspecific neck pain and related disability.

·         Workplace programmes combining ergonomic changes with neck-specific exercise have shown benefits for some office workers.
